Social Worker (Grant Per Diem Liaison/Case Manager)

The incumbent is responsible for monitoring and coordination of services provided by community-based programs funded under VA's Homeless Providers Grant and Per Diem Program.
Duties include:
Being responsible for a clinical program providing treatment to patients.
Managing daily operation of the program.
Developing policies and procedures for program operation.
Preparing reports and statistics for Facility, VISN, and national use.
Being responsible for the program's budget.
Having administrative responsibility for clinical program development and being accountable for clinical program effectiveness and modification of service patterns.
Implementing of VA's Health Care for Homeless Veterans (HCHV) Programs, Homeless Providers Grant and Per Diem (GPD) Program is authorized by Public Law 107-95, allowing VA to assist public or non-profit private organizations in establishing new programs by awarding grants, through a competitive process, for up to 65 percent of the cost of acquiring, renovating, or constructing facilities.
Ensuring that Veterans in community-based funded programs are receiving quality services.
Providing case management at Homeless Grant and Per Diem Program sites as described in the program rules, regulations, and directives.
Monitoring the clinical care and assessing the compliance of the programs receiving per diem as described in the grant recipient's original grant application.
Providing services and oversight of the Homeless Grant and Per Diem funded programs as defined in the program rules, regulations, and directives, to include verification of Veteran status and eligibility for participation in the program.
Coordinating and participating in program site inspections.
Coordinating care including serving as an advocate on behalf of the Veterans.
Coordinating with community agencies, organizations, and groups to learn of services or resources available for homeless Veterans.
Complying with criminal conflict of interest laws and Executive Branch Standards of conduct.
Participating in Medical Center research.
Participating in program planning and development.
Participating in orientation/teaching of graduate social work/nursing students, students of other disciplines, and Medical Center personnel on the biopsychosocial aspects of homelessness.
Improving professional skills.
Performing accountability functions.
Developing more effective and efficient ways of delivering healthcare services to Homeless Grant and Per Diem Program Veterans.
Providing case management at Homeless Grant and Per Diem Program sites.
